The Change of Matriculation (Change of Mat) process allows graduate students to make approved changes to their registration after enrollment. Refer to the Change of Matriculation Flowchart below for the step-by-step process and required procedures.
This section provides the process and requirements for graduate students who need to appeal for late registration. Please refer to the Late Registration Process Flowchart below for the step-by-step procedures and the required actions to complete your appeal.
PROCESSING OF APPEAL FOR LATE PAYMENT AND LATE REGISTRATION
STEP 1: The student submits a letter of appeal through OGS email at ogsche.upd@up.edu.ph, duly endorsed by the program adviser and the department Chair and must also indicate College Secretary’s name and designation see below:

STEP 2: The OGS Staff forwards the endorsed letter of appeal to the College Secretary for his endorsement.
STEP 3: The OGS Staff returns the endorsed letter of appeal to the Student, and the Student forwards the endorsed appeal to the OUR through email at our.diliman@up.edu.ph for approval.
STEP 4: The University Registrar acts on the appeal
STEP 5: The Office of the Chancellor acts on the appeal
STEP 6: Once approved, the OUR will process the Late Registration. The OUR will instruct the student to communicate with the College to facilitate the assessment of registration.
The Student must furnish OGS the copy of approval of appeal for Late Registration.
The OGS Staff will assess the student and then inform the student to proceed for payment.
*for the approved appeal for Late Payment proceed with the generation of payment slip via CRS.
STEP 7: Generation of Payment Slip
Go to “Settlement of Outstanding Transactions” module in your CRS account and do the following:
Generate a “Payment Slip” by clicking the "New Payment Slip" button;
Read carefully the instructions in the Payment Slip. Take note of the account number, account name, and the mode of payments that are allowed;
Pay your outstanding fee/s by the mode of payment of your choice;
Once your payment is successful, go back to the "Settlement of Outstanding Transactions" module in your CRS account and encode your payment details by clicking on the "Add Payment" button;
Fill-out the fields with appropriate details, then click on the "Save" button;
Wait for the Cashier’s Office to validate your payment. The average validation time is 3-5 days;
Once your payment has been validated by the Cash Office, you may print your copy of your Form5/Form5-Form/L/OA form (this is optional) using the same module. Click on the "Completed Transactions" tab, choose the semester, enter the correct academic year (make sure to complete the AY, e.g., 2021-2022 and not 21-22), and click on "Print Form" on the action column.

The Residence Registration process applies to graduate students completing their residency requirements while working toward their degree. Refer to the Residence Registration Flowchart below for the step-by-step process and requirements.
The following outlines the process for students applying for readmission from AWOL or requesting a waiver of the Maximum Residency Rule (MRR). Follow each step carefully and prepare all required documents before submitting your application.
STEP 1: Fill out the appeal for readmission and waiver of MRR form downloadable through this link.
STEP 2: Request OCG certification by visiting the Office of Counseling and Guidance (OCG) at the Vinzon’s Hall, 4th floor. Inform them of your intention for readmission or to appeal for the waiver of maximum residency rule. Fill out the necessary form/s that will be provided to you by OCG.
STEP 3: Submit your form along with your complete requirements through this link.
a. Letter of appeal addressed to the DEAN, Prof. Joanne R. Bantang, PhD to request for readmission or to justify the reason for inability to finish the degree program (if for maximum residence rule)
b. True Copy of Grades
c. Endorsement letter of the program adviser
d. Program of study
e. Endorsement of thesis adviser (as needed)
f. Thesis gantt chart (as needed)
STEP 4: The Student Record Evaluator (SRE) will assess your application. If all the necessary documents are complete, the SRE will forward it to the Department Chair for recommendation and signature.
STEP 5: If endorsed, the Department Chair will send back the documents to the Office of the College Secretary through the SRE for the deliberation of the College Graduate Council (CGC).
